Levetiracetam, marketed under the trade names Keppra among others, is a medication used to treat epilepsy. It is used for partial onset, myoclonic, or tonic-clonic seizures. It is the S-enantiomer of etiracetam.
Levetiracetam is available by mouth in two forms: immediate release and extended release.[3] It is also available by injection. An immediate release tablet has been available as a generic in the United States since November 2008, and in the UK since 2011. The United States patent for the extended release tablet will expire on September 17, 2028. It was initially patented by UCB Pharma.

Levetiracetam is very effective in animal models of generalized seizures induced by kindling and chemically. The exact anti-epileptic mechanism is still unknown, but it is different from traditional anti-epileptic drugs acting on ion channels or excitatory inhibitory neurotransmitter systems. The therapeutic concentration does not affect the synaptic conduction mediated by cetylbutyrate and glutamatergic neurons. The anti-epileptic mechanism may be: 1) inhibit the high-voltage activation of hippocampal pyramidal neurons per channel. 2) Acting on synaptic vesicle proteins to inhibit epileptic discharge by regulating the extracellular secretion function of synaptic vesicles and the release of presynaptic neurotransmitters. 3) Reverse the negative allosteric effect of the receptor antagonist, which inhibits the nerve and glyceric neurons and indirectly enhances the central inhibitory effect of glycine. 4) Block the down-regulation of cerebral cortex receptors and retain the down-regulated receptors in the hippocampus to enhance the inhibitory effect on neuronal circuits. The latest research shows that levetiracetam can also block the release of neurotransmitters before synapses, but the exact mechanism is unknown. It may be caused by binding to a certain protein selectively expressed on the presynaptic membrane of nerve terminals. 5) The receptors in the cerebral cortex can be regulated by reducing the current induced by the kainic acid salt gas-based via methoxalial and reducing the amplitude and frequency of the micro-excitatory postsynaptic current.
